I don't know if this applies to on-prem and cloud SaaS platforms, but with SaaS Siemplify recently began implementing a 3-month expiration on password reset requirements. If a user's password is not reset within 3 months, the system disables the user account. There are a couple of improvements that I'd recommend for this:

1) The password expiration timeframe should be able to be set by clients.

2) Users with an expired password should be able prompted to change their passwords before their accounts are disabled.

3) The user management page should display the date the password was last changed.
